Localizer planning

  • Acquire sagittal, coronal, and axial localizers centered at the glabella.

  • Cover the entire brain from left to right.
  • Cover from the top of the brain through the Foramen Magnum
  • Align parallel with the longitudinal fissure.

Axial planning

  • Cover the entire brain.
  • Align using the anterior and posterior aspect of the corpus callosum or use the AC-PC line, a line drawn from the upper surface of the Anterior Commissure (AC) horizontally to through the Posterior Commissure (PC) in sagittal view
  • Cover from the top of the skull through the foramen magnum.
  • For axial 3D T1 postcontrast imaging, include air above the scalp and extend inferiorly below the foramen magnum with balanced superior and inferior coverage.

Contrast indicated

  • Add axial 3D T1 postcontrast sequence.
  • Reformat axial 3D T1 postcontrast images in coronal and sagittal planes.

Surgical hardware present

  • Remove fat saturation from FLAIR.

Uncooperative patient

  • Consider switching to a fast or BLADE brain protocol.
  • When evaluating possible CVA in an uncooperative patient, acquire DWI before other axial sequences.
