MSK protocol

Thumb MRI

MRI of the thumb using a small extremity coil. Begins with wide-field coronal STIR and T1 of the hand for lesion localization, then acquires focused sagittal, axial, and coronal sequences through the entire thumb. Coronal alignment is through both sesamoids. Axial coverage runs from the first carpometacarpal joint through the tip of the thumb.

For protocol planning and internal reference only. Follow institutional MRI safety screening, contrast, dosage, and GFR policies.

sagittal

Sagittal planning

  • Prescribe plane perpendicular to the coronal imaging plane.
  • Scan through the entire thumb.

axial

Axial planning

  • Prescribe plane perpendicular to the midshaft of the proximal phalanx of the thumb.
  • Cover from the first carpometacarpal joint through the tip of the thumb.

coronal

Coronal planning

  • Prescribe plane parallel through both sesamoids.
  • Scan through the entire thumb.
Anatomic extentCoverage RulesMinimum coverage expectations and alignment rules.

axial

Axial field of view

First carpometacarpal joint through the tip of the thumb, aligned perpendicular to the midshaft of the proximal phalanx.

General

Coronal and sagittal field of view

Entire thumb. Coronal aligned parallel through both sesamoids.
